In some cases, following a review with your general practitioner along with a psychiatrist or paediatrician you may then be eligible to receive a further 20 rebated appointments, issued in lots of 10.
Please note. We recommend that you consider booking an appointment with your specialist consultant (psychiatrist or paediatrician) after your first Eating Disorder Treatment Plan review with your general practitioner, as many specialist consultants have long wait times for appointments. This review can be completed any time between session 11 & 20.
Under this plan you can access a total of 40 Medicare rebated appointments, which are issued in lots of 10 appointments. This plan is active for a 12-month period from the plan issuing date.
Chronic Disease Management Plan
Medicare introduced this plan to offer support for people with chronic or terminal medical conditions.
Chronic medical conditions are those that have been, or are likely to be present for at least six months. This includes asthma, cancer, cardiovascular disease, diabetes, kidney disease, musculoskeletal conditions, and stroke.
There is no list of eligible conditions, however, the Chronic Disease Management Plan is designed for people who require a structured approach to managing their condition, including those requiring ongoing care from a multidisciplinary team.
If your chronic or terminal medical condition requires the care from a multidisciplinary team, from your general practitioner and at least two other health care providers then you are eligible for Medicare rebates for certain allied health services such as psychology upon a referral from your general practitioner.
A referral is valid for the stated number of services. If all services are not used during the calendar year, the unused services can be used in the next calendar year however, those services will be counted towards the five rebates for allied health services available to you during that calendar year.
It is possible to receive up to 5 rebated appointments of $60.35 each under the Chronic Disease Management (CDM) Plan if your general practitioner determines that you are eligible and best managed under this plan.
